Building systems that do not depend on one person

Excellent care often rests on one committed doctor or nurse — and becomes inconsistent the moment that person is unavailable. That is rarely a problem of commitment; it is a problem in the system around the individual.

I map how care is actually delivered, clarify roles and escalation, design practical processes, train the team that owns them, measure performance — then hand over, so the improvement continues without me. The aim is not to remove professional judgement, but to give it a reliable foundation.

Neonatal intensive care unit with an incubator and monitoring equipment

Building neonatal services

Establishing and developing Level III neonatal services across different healthcare organisations: clinical planning, recruitment, training, protocol development, neonatal transport, quality improvement, and building teams capable of taking increasing ownership.

Improving care across hospitals

Quality-improvement networks and collaborative programmes with teams across Karnataka and South India, addressing Kangaroo Mother Care, breast-milk use, infection prevention, antimicrobial resistance and healthcare-worker wellbeing. Several have resulted in peer-reviewed publications.

Developing women's and children's services

Currently developing an integrated women's and children's hospital in Bengaluru: clinical leadership, service planning, workforce development and the operational systems that hold a new hospital together.

Extending care through technology

Founder of Umbel Care and developer of Neocleus, a neonatal-specific electronic medical record used across hospitals in four Indian states; later Chief Medical Officer of a telehealth organisation.

Ideas and frameworks

A significant part of my work involves converting experience into practical tools that healthcare teams can use — including The Child Experience, a framework for looking at healthcare through the eyes of a child; Lessons From Practice, short reflections on the system issues beneath everyday clinical situations; and practical frameworks for nursing care systems and clinical pathways.
